The Delaware Health Equity Coalition is the vision and mission of three physicians who fought together on the frontlines of the Covid pandemic and heard the cry of many patients who felt dismissed by many in the healthcare sector, and quite often disrespected.  These courageous conversations coupled with more than 80 years of medical service prompted our mission to change the way health care is delivered to struggling communities in Delaware.

• All care providers will serve not only as messengers of truth, but agents of change to alleviate healthcare inequities, mental health challenges and health-related social needs
• The Health Equity Dashboard and key performance metrics will ensure appropriate priorities are set and targeted healthcare needs are met
• Individual and community health is measured as the sum total of mental, physical, and spiritual well-being
• Leverage technology that will target, track, and guide treatment to achieve healthcare outcomes
• Collaborate with community leaders, local organizations, and educational institutions to develop programs that ensure the mission is reaching all black and brown communities
• Continue the legacy of being a source of refuge and restoration in the community
• Through the lens of historical heritage, we will connect to the community one patient at a time with empathy, compassion, and clinical excellence
