The Delaware Health Equity Coalition is the vision and mission of three physicians who saw the need to change the way health care is delivered to the struggling communities in Delaware.  While being proximal and intentional, we will deliver care to our patients in a manner that they expect: to be seen, heard, and treated with respect because we care. 

• All care providers will serve not only as messengers of truth, but agents of change to alleviate healthcare inequities, mental health challenges and health-related social needs
• The Health Equity Dashboard and key performance metrics will ensure appropriate priorities are set and targeted healthcare needs are met
• Individual and community health is measured as the sum total of mental, physical, and spiritual well-being
• Leverage technology that will target, track, and guide treatment to achieve healthcare outcomes
• Collaborate with community leaders, local organizations, and educational institutions to develop programs that ensure the mission is reaching all black and brown communities
• Continue the legacy of being a source of refuge and restoration in the community
• Through the lens of historical heritage, we will connect to the community one patient at a time with empathy, compassion, and clinical excellence
