The Delaware Health Equity Coalition is a patient-focused medical home that provides comprehensive healthcare that educates, empowers, and engages our patients in a manner that fosters empathy, compassion, and trustworthiness. We are in the heart of Wilmington, Delaware, so our patients are neighbors and friends, not numbers and data files.

Our PCPs provide the full scope of primary care services, including diagnosis, treatment, coordination of care, preventative care, and health maintenance to patients. They serve as leaders within the patient-centered medical home model and work collaboratively with the care team to ensure that each patient gets the best care they deserve.

Maintain confidentiality and protect the Practice by abiding by laws and principles related to confidentiality; keep information concerning practice operations, patients, and employees confidential.

This position requires compliance with DHEC’s compliance standards, including its Standards of Conduct, Compliance Program, and policies and procedures. Compliance will be an element considered as part of the Primary Care Physician’s regular performance evaluation.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Participates as an active clinical staff member and participates in daily care team huddles to ensure prioritization of patient needs.
  • Attends to patients promptly and orderly to keep waiting time to a minimum and maximize productivity.
  • Obtains appropriate patient history, performs physical evaluation, orders appropriate diagnostic tests, and follows up with interpretation. Formulates assessment and initiates treatment plan with appropriate follow-up appointments.
  • Reviews incoming reports (e.g., lab, x-ray, EKG), signs, and dates and follows up with patients in a timely manner.
  • Ensures documentation is recorded in a complete, timely, and orderly manner, utilizing templates in the electronic medical record.
  • Must be willing to take call in a rotation
  • Consult and refer with/to appropriate Specialty Physicians.
  • Participates in the quality care review and chart review program.
  • Assesses, stabilizes and triages patients.

EXPERIENCE AND SKILL REQUIREMENTS:

The following experience and skills are considered essential:

  • A minimum of at least three years of experience in the outpatient setting is preferred.
  • Board certification/board-eligible Family Medicine or Internal Medicine without OB
  • Full-time position preferred (36 direct patient hours with 4 hours administrative hours)
  • If coming from another organization, you must have tail coverage from a prior organization.
  • Ability to handle multiple priorities.
  • Ability to deal with trauma situations and death.
  • Ability to make difficult decisions.
  • Strong aptitude for calculating correct medicine doses.

EDUCATION REQUIREMENTS:

The following education requirements are considered essential:

  • Medical Degree (MD or DO) from an accredited medical school
  • Completion of a residency program in Internal Medicine or Family Practice
  • Board-certified in Internal Medicine or Family Medicine
  • Current state medical license
  • Current state Federal DEA license
  • Continuing education to maintain licensure to enhance the community’s health and provide excellent patient care to a full spectrum of constituents.
